I am having the same issue as trouble-buster. If a customer living in Nevada (where 8.1% tax should be added) puts in their state and zip code, this is the message that comes up: "Please specify shipping location below to calculate your shipping costs Please provide a Zipcode and click Calculate in order to continue." I tried turning off the billing and shipping form, but that doesn't work. My settings are: tax exclusive, add per product tax, apply tax when shipping country is the same as tax rate.
